IN HOME SUPPORT
Life @ Home, LLC provides in home care ranging from
supportive guidance and encouragement in independent living to direct
assistance with personal cares.
Based on an individual’s needs, Life @ Home, LLC will develop a
comprehensive care plan that addresses the extent of assistance and
care the individual needs including household maintenance, meal planning
and preparation, cleaning and laundry, personal hygiene, and day to
day living.
COMMUNITY ACCESS SUPPORT
Life @ Home LCC provides supports to access daily
and event driven community activities such as shopping, dining out,
appointments, social activities, worship services, festivals, and
other community opportunities.
Whether it’s as simple as accompanying and guidance or as complex
as supporting mobility and physical support systems, we will provide
the support an individual needs to have full access and enjoyment
of community resources and activities.
COMPREHENSIVE NEEDS ASSESSMENT
Life @ Home, LLC will provide individuals with a FREE
comprehensive needs assessment that includes medical, intellectual,
physical, social, and support needs.
The assessment will include past, present, and future needs and may
involve input from family and medical personnel.
